- Reappointment Go Live for 2024/25 is the week of February 19, 2024 – that means you will receive an email from CMaRS initiating the re-appointment process. Deadline for completion is March 31, 2024. This deadline is firm.
- The courses listed below are mandatory and will be accepted if completed any time after January 1, 2024.
- A Physicians Guide for Accessing the LMS for Re-Appointments is attached

- In addition to the CMaRS online re-appointment form, Medical staff will also be required to complete the Relationship Management form via CMaRS. For questions about Relationship Management please contact Marianna Betro at 416-864-5521 or [email protected]
- Attestations are required for:
- 2023 Proof of Continuing Medical Education-CME (Royal College or College of Family Physicians or College of Dentistry)
- COVID Vaccine – two (2)doses
- Current insurance coverage
- Required payments on the CMaRS system include where applicable:
- CMaRS Re-appointment Fee ($50 if applying at a single institution, $75 if applying to more than one institution)
- SMH Active Staff ($200) and Courtesy Staff ($75) MSA dues (SMH Clinical Associates and Midwives do not pay MSA dues)
- SJHC Active Staff ($500) AND Courtesy and Midwifery MSA dues ($150) (SJHC Clinical Associates do not pay MSA dues)
- PHC MSA dues of $250
- Itemized receipts are generated by Chase Bank & emailed to the physician as soon as the transaction is processed (if an email is entered when paying)
- CMaRS cannot re-produce these receipts
- It is the physician’s responsibility to keep track of their receipts
