ALC Designations

Alternate Level of Care

  • Patients to designate ALC:
    • Medically stable
    • Awaiting placement in a nursing home, chronic care facility, or rehabilitation facility.
    • Placed on a waiting list for transfer to Bridgepoint Hospital or Providence Health Centre
  • These patients should be rounded on twice per week, or more frequently if new medical issues arise
  • Once a patient is made ALC, you must:
    • Put the order in on Soarian
    • Notify your case managers
  • Try to have discharge summaries partially completed when ALC designation is made – as we are often notified of rehabilitations beds on the day of transfer!
